Constructs a writer from derived key material.

`cipherKey`	AES-CTR key (16 bytes for aes128-ctr, 32 for aes256-ctr).
`iv`	16-byte initial counter block.
`macKey`	HMAC key (SHA-256/512 size).
`mac`	MAC algorithm.
`rng`	Random source for padding bytes.

SshEtmException on unsupported key sizes.
